研究人员在三个血液学实验室进行了为期4个月的多中心前瞻性研究。在步骤1中，使用一个自动化的信息学系统来筛查137453例贫血和/或大细胞增多症患者的血象(n = 2702)。在步骤2中，排除了所有贫血似乎是由已知原因引起的患者。这导致290名患者贫血病因不明。在第三步，我们进行了进一步的调查，包括外周血涂片，并分析铁、维生素B12、叶酸和甲状腺激素水平。

本研究对139例(48%)患者进行了鉴别诊断。贫血的主要原因是缺铁(59例)和巨幼细胞性贫血(39例)。确诊血液病25例，其中MDS 14例(56%)。MDS患者的中位年龄为80岁，其中12例贫血为孤立性细胞减少症，大多数(n = 10)患者的风险较低(IPSS‐R≤3.5)。SF3B1突变最常见(n = 6)，且与成核细胞的存在相关(100%)，与更好的预后相关(P = 0.001)。

研究表明，本研究前瞻性的四步法是一种有效的、符合逻辑的策略，有助于基于不明原因贫血和/或大细胞增多症的MDS诊断，并可能使最严重的贫血原因的早期诊断成为可能。对MDS相关基因的分子分析可能是一种有前途的诊断和预后方法。
