非布司他的临床用药,掌握这5点就够了!

非布司他

非布司他通过抑制黄嘌呤氧化酶(XO),而抑制尿酸的生成。别嘌醇能够抑制还原型XO,而非布司他对还原型、氧化型均有很好的抑制作用,因此降尿酸作用更强大,更快速。

非布司他适用症非布司他适用于有痛风病史,或者在持续高尿酸血症中发作痛风的患者长期使用。也推荐在痛风发作急性期配合抗炎的药物同时联合使用降尿酸。


不过,中国的说明书和指南共识都不推荐它用于无临床症状的高尿酸血症患者。

FDA仅推荐它用于使用别嘌醇治疗无效、或使用别嘌醇发生严重不良反应的患者。

非布司他服用方法非布司他有10mg、20mg、40mg和80mg多种规格,服药之前要注意查看自己服用的是哪种规格。

*推荐初始剂量为20mg每日1次在服药2~5周后复查血尿酸,根据血尿酸值逐渐增加用量,每次增量20mg,每日最大剂量为80mg。血尿酸值达标后,维持最低有效剂量。食物和抗酸剂对服药没有影响。

*在治疗初期,可能因为血尿酸值急速降低导致组织中沉积的尿酸盐被动员出来更多,导致痛风发作频率增加。

*预防治疗初期的痛风发作建议从小剂量开始逐渐加量并在医生指导下使用秋水仙碱以及非甾体抗炎药(例如布洛芬等)

*有痛风急性发作的患者,在使用非布司他前,需先控制痛风症状。若在非布司他治疗期间痛风发作,则无需中止非布司他的治疗。应根据发作的具体情况,对痛风进行相应治疗。

注意事项

*非布司他对XO的抑制作用可能会升高通过此酶代谢的药物如硫唑嘌呤、巯嘌呤、茶碱的血药浓度,从而产生毒性,所以禁用于正在使用这三种药物的患者

*需要提醒大家的是,非布司他可以控制痛风但不能治愈痛风。痛风患者降尿酸治疗是一个长期的过程,血尿酸降至达标后不能随意停药,需根据尿酸水平遵医嘱减量或停药。

擅自停用降尿酸药物易导致血尿酸恢复至治疗前水平,不仅可再次引起痛风发作,且血尿酸长期不达标还会带来肾脏和心脑血管受损的风险。

药物治疗是治疗痛风的重要措施,但生活方式的改变也必不可少。

*生活方式的改变包括:减少高嘌呤食物的摄入,心肾功能正常者多饮水(最好每天2000毫升以上),同时戒烟, 禁啤酒和白酒。不得不喝酒的话,红酒可适量。还要控制体重。

非布司他的不适用人群

►对非布司他片成份有过敏史的患者。

►正在使用巯嘌呤或硫唑嘌呤的患者,因为非布司他可能引起硫唑嘌呤、巯嘌呤和茶碱的血药浓度升高,毒性增加。
►目前没有非布司他用于继发性高尿酸血症患者的研究,因此,继发性高尿酸血症患者不建议使用。
►无症状的高尿酸血症患者不建议使用。

非布司他的不良反应及处理措施

对黄嘌呤氧化酶之外的酶,非布司他不会产生抑制作用,因此,它安全性相对较高,但也要注意其不良反应。*非布司他主要在肝脏中代谢,代谢后的非活性物质通过肾脏、粪便还有胆汁多途径排泄,非布司他减少肾间质纤维化形成,发挥肾脏保护作用。非布司他可能具有延缓或预防 CKD 患者肾间质纤维化的作用。

 

*肝功能损害:发生率 2%~3%,表现为疲劳、食欲不振、右上腹不适、酱油色尿、黄疸、腹泻等情况。
处理措施首次使用非布司他之前,患者应检查肝功能作为基线水平。用药时

出现上述肝功能损害出现的症状时,应检查肝功能。
肝酶轻度升高(低于正常参考值上限 2倍)可随访观察, 多数可逐渐恢复正常。肝酶升高超过正常参考值上限 2倍时,建议给予保肝治疗, 超过正常参考值上限 3倍时,则需要酌情减量或停药,一般经过停药或保肝治疗,肝功能受损症状均可缓解。

*在肾功能不全和肾移植患者中具有较高的安全性,轻、中度肾功能不全(eGFR≥30ml/min)者无须调整剂量。中外高尿酸血症相关诊治指南对于慢性肾脏病患者的高尿酸血症的治疗,都在一定程度上更建议使用非布司他。对于重度肾功能不全(eGFR<30 ml/min)的患者,已有多项研究显示非布司他的有效性及安全性,建议起始剂量为20mg,每日 1 次。

处理措施:轻度或中度(Child-Pugh A 和 B 级)肝功能不全者无须调整剂量,尚无重度肝功能不全(Child-Pugh C 级)的充足研究数据,此类患者应慎用。

*皮肤反应:表现为皮肤过敏反应、皮疹、瘙痒、水疱、脱皮等。
处理措施:如怀疑发生严重的皮肤反应,应中止使用非布司他,及时就医。若患者曾在使用别嘌醇时报告过类似的皮肤反应,则应慎重使用非布司他。*心血管疾病相关的死亡:有研究表明,和别嘌醇相比,非布司他导致的心血管相关死亡增加。

此研究目的是提醒医生和患者更合理的使用该药,并不是全盘否认它的疗效,这也是FDA限制它仅用于使用别嘌醇治疗无效、或使用别嘌醇发生严重不良反应的患者的原因。

处理措施:在决定使用非布司他时应充分评估,对存在心血管疾病病史或危险因素的患者需谨慎使用,并对患者随访监测,警惕心血管血栓事件的发生。
对于有心脏病或中风病史的患者,需要仔细权衡风险和获益。

在服用非布司他期间若出现胸痛、呼吸急促、心跳过快或不规则、身体一侧麻木或虚弱、头晕、说话困难、突发性剧烈头痛等症状,应立即就医。

 

特殊人群使用事项

 

FDA 妊娠安全分类为C类:在孕妇中没有进行过充分的对照研究。只有确认潜在益处大于对胎儿的风险时,妊娠期间才能使用非布司他。

非布司他的哺乳期安全分级为L3级。对大鼠的研究发现,非布司他可经乳汁排泄。但尚不知道非布司他是否会经人乳排泄。

由于很多药物可分泌到乳汁,因此,哺乳期妇女应慎用本品。哺乳期妇女使用本品给药期间应停止哺乳。

尚未确定非布司他治疗18岁以下患者的安全性和有效性,18岁以下儿童不推荐使用。

老年患者无需调整剂量。 

总结 

非布司他作为新型黄嘌呤氧化酶抑制剂,具有明确的降尿酸作用,为痛风和高尿酸血症患者的治疗提供了用药选择。在临床应用非布司他的过程中,应警惕该药不良反应及风险,并应注意防范黄嘌呤氧化酶抑制剂停药综合征。目前国内风湿免疫科医师针对痛风、高尿酸血症患者在考虑进行非布司他降尿酸治疗时,应权衡风险与获益,依据患者的具体情况判断,客观地评价药物的临床使用意义。
