人们对创伤性脑损伤导致进行性神经功能恶化的可能性非常感兴趣。虽然胶质纤维酸性蛋白和神经丝光等血液生物标志物已被广泛用于表征急性创伤性脑损伤,但它们在慢性期的使用受到限制。鉴于越来越多的证据表明这些蛋白质可能是一系列疾病中持续神经退行性变的标志物。

Virginia F. J. Newcombe等在BRAIN杂志发表题为Post-acute blood biomarkers and disease progression in traumatic brain injury研究文章,检查了胶质纤维酸性蛋白和神经丝轻链水平与创伤性脑损伤后数月至数年的成像变化和功能结果的关系。

患者队列的数据与 59 名健康志愿者和 21 名非脑损伤创伤对照进行了比较。计算皮质灰质、深部灰质和全脑白质的平均扩散率和分数各向异性。

脑创伤后5年以上的患者和对照组的灰质(图A)和白质(图B) VBM对比结果。

在受伤后8个月到5年之间显示改善(GOSE升高1分),稳定(GOSE没有变化),或恶化(GOSE减少1分)的亚组患者中全脑灰质和全脑白质 (图A和B)和GFAP和NFL水平(图C和D)的预测脑龄差异。

该研究发现:

  1. 慢性早期血清神经丝轻链浓度升高。虽然8 个月时胶质纤维酸性蛋白(GFAP)值处于正常范围内,但许多患者在受伤后超过 5 年表现出继发性和暂时性明显升高。
  2. 6个月时的生物标志物升高与弥散张量成像的微观结构损伤指标显着相关。
  3. 约 8 个月的生物标志物水平预测5年以后的白质体积减少,以及8 个月-5 年的年化脑容量减少。
  4. 在 8 个月-5 年之间功能恶化的患者表现出高于预期的脑年龄和升高的神经丝轻链水平。
