J CACHEXIA SARCOPENI:慢阻肺患者口服β-丙氨酸12周后的疗效

补充β-丙氨酸可有效增加 COPD 患者的肌肽,然而,未观察到伴随的运动能力、股四头肌功能和肌肉氧化/羰基应激的有益变化。

补充β-丙氨酸 (BA) 可增加肌肉肌肽,肌肽是骨骼肌中丰富的内源性抗氧化剂和 pH 缓冲液。肌肽负荷可促进健康老年人的运动能力。由于慢性阻塞性肺疾病 (COPD) 患者存在运动诱发的肌肉氧化/羰基应激和酸中毒,以及肌肉肌肽储备减少,Journal of Cachexia, Sarcopenia and Muscle杂志发表了一项研究,调查了补充β-丙氨酸是否会导致COPD患者增加肌肉肌肽,并诱导运动能力、股四头肌功能、肌肉氧化/羰基应激的有益变化。

40例 COPD 患者(75%为男性)(年龄65±6岁;FEV1%预测值55±14%)被分组接受12周口服β-丙氨酸或安慰剂补充剂(3.2 g/天)。主要结局,即肌肽,通过干预前后的股外侧肌活检来量化。还评估了协同主要结局,即增量和恒定的工作速率周期能力。进行了线性混合模型分析。还评估了补充剂摄入的依从性和副作用以及次要结局(股四头肌力量和耐力,以及肌肉氧化/羰基应激)。

结果显示,与安慰剂相比,补充β-丙氨酸增加了 COPD 患者的肌肉肌肽(平均差异 + 2.82[1.49~4.14]mmol/kg湿重;P < 0.001)。最大增量循环能力(VO2peak:+ 0.5[0.7~1.7]mL/kg/min;P = 0.384,Wpeak:+ 5[1~11]W;P = 0.103)和恒定功率循环试验的力竭时间(+ 28[179~236]s;P = 0.782)无显著变化。

β-丙氨酸组(中位数,100(98~100)%)和安慰剂组(98(96~100)%)(P = 0.294)对补充剂摄入的依从性相似,患者未报告可能与补充剂摄入相关的副作用。次要结局无变化。

与安慰剂相比,补充β-丙氨酸可有效增加 COPD 患者的肌肽(较平均基线值 + 54%),且无副作用。然而,未观察到伴随的运动能力、股四头肌功能和肌肉氧化/羰基应激的有益变化。
