输液反应抢救大全！

静脉输液治疗是现代药物治疗的重要手段，在治疗某些疾病和挽救患者生命方面有着不可替代的作用，而另一方面，静脉输液属有损操作，药物直接入血，与其他给药途径比较，其风险相对较高，不合理使用输液会增加严重不良

静脉输液治疗是现代药物治疗的重要手段，在治疗某些疾病和挽救患者生命方面有着不可替代的作用，而另一方面，静脉输液属有损操作，药物直接入血，与其他给药途径比较，其风险相对较高，不合理使用输液会增加严重不良反应的发生风险。
今天主要和大家分享的是以下几种常见的输液反应，包括药物热原反应、静脉炎、容量负荷过重、药物过敏，尤其是过敏性休克。

药物热原反应

药品生产本身带有或输液器材、输液操作不当都可引入致热原，主要是细菌的内毒素，当超过限度，往往可出现发热反应。
临床表现：
发冷、寒战、面部和四肢发绀，继而发热，体温可达40℃左右；可伴恶心、呕吐、头痛、头昏、烦躁不安，老年人还可能出现心衰、死亡等。
抢救原则：
1. 停止输液，更换输液器。
2. 适当补液。
3. 降温：物理降温，药物降温。
4. 立即注射非那根（25～50 mg）或其他抗过敏药物，留观病人待退热至38℃以下，并无其他不适才可离开。如发生休克时，按过敏性休克抢救流程进行抢救。

静脉炎

对于有刺激性的药物，如果药物的浓度、酸碱度、渗透压过高，输液速度过快等，药液对血管会产生刺激，局部出现炎症反应。
临床表现：
局部发红、肿胀、灼热、疼痛，可有全身症状如畏寒、发热等，沿静脉走向可发现条索状的红线。
抢救原则：
1. 减慢、停止输液，更换注射部位
2. 抬高患肢
3. 理疗或热湿敷（95％酒精或50％硫酸镁）

容量负荷过重

输液过量或过快，特别是输入含钠液体过多时，容易发生急性左心衰，原有心脏病，心功能不全者，肺功能不全者，老年人、儿童输液应特别注意。
临床表现：
发病时患者突然感到呼吸困难、气促、剧烈咳嗽、烦躁不安、口唇发绀，严重时口鼻可喷涌出大量粉红色泡沫样液，听诊两肺出现干湿性音，心音弱速。
抢救原则:
1. 立即停止输液，取端坐位，两腿下垂，以减少静脉回流，减轻心脏负担。
2. 立即高流量吸氧。湿化瓶给予20%～30%酒精，湿化吸氧。酒精能降低肺泡内泡沫表面张力，使其破裂消散，改善肺部气体交换，迅速缓解缺氧症状。
3. 强心、利尿、扩管：西地兰、速尿、硝普钠等。
4. 平喘：滴注氨茶碱。

药物过敏反应

50%的过敏性休克发生于输液后5 min内，40%发生在用药后20 min内；通常发生越早，症状越重。
临床表现：
荨麻疹，皮肤潮红、骚痒；头痛、焦虑不安，意识障碍；胸闷、憋气；恶心、呕吐、腹痛；出冷汗、低血压等。致死原因主要是喉头水肿引起窒息和休克。
过敏性休克的特征就是喉头水肿+哮鸣音+过敏药物。
抢救原则：
1. 停止原有可能过敏的输液，更换输液管；快速输液如盐水、林格注射液。
2. 平卧，吸氧，保暖。
3. 抗过敏药物：葡萄糖酸钙1 g稀释后缓慢静脉注射；抗组胺药如苯海拉明；糖皮质激素治疗如地塞米松、氢化可的松等。
4. 保持气道通畅，必要时气管插管。
5. 支气管扩张剂：氨茶碱。

过敏性休克

临床诊断：
    
1. 有过敏接触史；
    
2. 表现胸闷、喉头堵塞感、继而呼吸困难、紫绀、濒死感，严重者可咳出粉红色泡沫样痰；
    
3. 常有剧烈的肠绞痛，恶心、呕吐、或腹泻；
    
4. 意识障碍，四肢麻木、抽搐、失语、大小便失禁、脉细弱、血压下降。
抢救要点：抢救需要迅速、及时、就地抢救。
抢救措施：
1. 除去过敏原，如静脉用药，换掉输液器和管道，不要拔针，接上生理盐水快速滴入。置患者于平卧位、给氧。
2. 首选肾上腺素，0.1%肾上腺素0.25~0.5毫升肌注或皮下注射，可视病情20-30分钟重复使用。肾上腺素是首选药物，但不能应用心肺复苏的剂量（如1 mg静脉 注射）来抢救过敏性休克，因为心肺复苏是骤停心律，而过敏反应是有灌注。
3. 静脉快速注入肾上腺皮质激素：地塞米松10~20毫克加5%葡萄糖100毫升（静滴）。
4. 吸氧或高压给氧。
5. 扩容：平衡液：500~1000毫升静滴，必要时使用升压药。
6. 给予钙剂及抗组胺药物：10%葡萄糖酸钙20毫升，静脉缓注。
7. 平喘 ：氨茶碱0.25克加50%糖40毫升静脉缓注；呼吸抑制可以用呼吸兴奋剂。
8. 心脏骤停需要马上进行胸外心脏按压。
另外，还要提醒各位，在患者进行输液过程中要经常巡视，随时注意观察其状态，如有可疑反应症状应立即暂停或更换液体，严密观察并及时采取必要的相应措施，才能确保病人的安全。
 牢记：53种疾病不需要输液 
静脉输液是公认最危险的给药方式。据不完全统计，我国每年输液量超过100亿瓶，每年约有20万人死于输液不良反应。专家指出，如果患者肠胃功能正常，口服药物应是第一选择。
