肾素-血管紧张素系统（RAS）信号传导和血管紧张素转换酶2（ACE2）涉及到急性呼吸窘迫综合征（ARDS）的发病机制。近期，一项发表在杂志Crit Care上研究评估了使用GSK2586881（一种重组形式的人血管紧张素转换酶2（rhACE2））来补充ACE2是否可以减轻急性肺损伤。研究者们进行了两部分II期试验，其中包括在北美的十个重症监护病房中开放标签的门诊剂量升高阶段和随机、双盲、安慰剂对照

研究者们进行了两部分II期试验，其中包括在北美的十个重症监护病房中开放标签的门诊剂量升高阶段和随机、双盲、安慰剂对照阶段。受试患者年龄在18至80岁之间，有美国-欧洲ARDS共识标准的一致性诊断，且机械通气不到72小时。在A部分中，开放标签GSK2586881以0.1mg/kg至0.8mg/kg的剂量施用并评估安全性，药代动力学和药效学。在对A部分资料进行回顾之后，进行了3天的每日两次剂量GSK2586881（0.4 mg/kg）的随机、双盲、安慰剂对照调查（B部分）。在给药期和随访期间收集生物标志物，生理学评估和临床终点事件。

此项研究结果显示：在A部分中，患者对剂量升高具有良好的耐受性，无临床显着的血液动力学变化。B部分的无效分析后，计划60名患者中的39名被终止。输注GSK2586881后，血管紧张素II水平迅速下降，而血管紧张素（1-7）和血管紧张素（1-5）水平升高，并且持续升高48 h。与安慰剂相比，表面活性剂蛋白D浓度增加，而rhACE2治疗受试者的白细胞介素-6浓度具有下降趋势。动脉氧分压与吸入氧分数，氧合指数或序贯器官衰竭评估评分的比值无明显差异。

此项研究表明：GSK2586881在ARDS患者中具有良好的耐受性，RAS肽的快速调节表明目标靶点的参与。
