根据1984年发表的阿尔茨海默病(AD)的第一个诊断标准,只有在死后才能对AD进行明确诊断。 对AD病理生理学的理解和技术进步有利于开发评估AD生物标志物的新技术,实现不仅在体内而且在临床症状发作之前检测AD。因此,AD生物标志物已被逐步引入临床实践,其临床应用也在不断增长,这与承认其关键病理生理意义的最新诊断标准一致。AD生物标志物可分为三大类:“A”指β-淀粉样蛋白,“T”指tau,“N”指神经退行性变。它们可以通过不同的技术进行评估,如神经成像(即MRI和正电子发射断层扫描(PET))、脑脊液(CSF)或血浆。每种技术都有优点和缺点。具体而言,神经成像技术可以直接测量AD生物标志 但其信息仅限于单个生物标志物(即仅a、T或N),成本更高,有辐射。CSF分析是一种更便宜、更容易获得的技术,可以同时提供所有ATN(和其他)生物标志物的信息,但它是一种间接测量,不能提供具体的解剖的信息,并且由于其侵入性,有时不被患者接受。血浆与脑脊液具有相同的优势,但其更高的可及性可能使在不久的将来进行大规模的AD筛查成为可能,并通过更定期的重复采样监测疾病进展。

一些神经成像技术(即结构MRI和氟脱氧葡萄糖(FDG)-PET)和CSF测量已用于临床目的很长一段时间,其临床有效性得到了一致证据的支持。 最近开发了允许评估淀粉样蛋白和tau沉积的PET示踪剂,初步证据表明其临床有效和诊断影响。 因此,神经成像和CSF测量可以被认为是评估AD生物标志物的传统技术/检查。不同的是,血浆生物标志物最近才出现, 而且关于其临床有效性的证据尚不可用。然而,鉴于血浆相对于神经成像和脑脊液生物标志物的优势,迫切需要证明其临床有效性及其在筛查记忆临床患者方面的表现。本研究的目的是:(1)确认血浆和同源的传统AD生物标志物之间的相关性,(2)评估血浆生物标志物与传统生物标志物相比的诊断准确性。研究发表在《神经病学,神经外科学和精神病学杂志》上()。

患者因认知障碍来到该中心,并接受了诊断检查,包括临床和神经心理学评估,以及与临床相关或作为研究项目一部分的生物标志物调查。患者按认知阶段(认知未受损(CU)、轻度认知障碍(MCI)、痴呆)进行分类。神经成像和脑脊液测量被认为是传统的生物标志物。神经成像、脑脊液和血浆生物标志物分为淀粉样蛋白、tau和神经退行性变类别。比较了测量与神经退行性变相关的不同病理生理过程的生物标志物,即:萎缩、低代谢、轴突损伤和星形胶质细胞增生。评估了血浆Aβ42/Aβ40作为淀粉样蛋白的测量值,与之前的研究一致,之前的研究报告称,该测量值比单独的Aβ42更能识别淀粉样蛋白沉积。血浆p-tau/Aβ42是淀粉样蛋白和tau的测量值。血浆Aβ42、Aβ40和NfL浓度是根据试剂盒制造商(Quanterix,Billerica,MA)的说明,在HD-X分析仪上使用市售的单分子阵列(Simoa)测定法测量的。获取结构3T MRI图像。左右海马体积的平均值(使用FreeSurfer(V.7.0)recon all工具的自动分割提取),通过颅内总体积进行调整,用作连续变量。

浆生物标志物与通过传统技术评估的生物标志物显著相关:淀粉样蛋白的r=0.50(p<0.001),tau的r=0.43(p=0.002),神经退行性变生物标志物的r=−0.23(p=0.001)。此外,血浆生物标志物在区分通过使用传统生物标志物确定的生物标志物状态(正常或异常)方面显示出很高的准确性:淀粉样蛋白的曲线下面积(AUC)=0.87,tau的AUC=0.82,神经退行性变状态的AUC=0.63。 血浆NfL与传统生物标记物(MRI和FDG-PET)之间的相关性较弱。此外,血浆生物标志物在检测传统淀粉样蛋白和tau生物标志物阳性方面达到了极好的诊断准确性(血浆Aβ42/p-tau比值显示出最佳性能)。

血浆生物标志物可以节省的传统检查的比例

血浆p-tau(单独使用和与Aβ42联合使用)在与传统生物标志物的相关性和诊断准确性方面均优于传统生物标记物,而单独使用Aβ42和NfL与淀粉样蛋白和神经退行性变传统生物标志剂的相关性较弱,诊断准确性较差。此外,血浆生物标志物被证明是检测记忆临床患者淀粉样蛋白和tau状态的有用工具,而它们检测神经退行性变状态的潜力似乎不足。这可能是由于神经退行性变及其成像标志物的非特异性和更异质性,而淀粉样蛋白和tau沉积与特定神经退行性疾病(如AD)的存在更密切相关,这些疾病与血浆生物标志物有关。

血浆生物标志物诊断AD,可以代替传统的诊断方式,更具成本效益,并改善患者护理。
