Blood:精氨酸治疗对儿童镰状细胞病血管闭塞性疼痛期间线粒体功能的影响

静脉精氨酸治疗可增加SCD和血管闭塞性疼痛患儿的线粒体活性,并降低氧化应激; 精氨酸治疗对线粒体功能的剂量依赖性影响是一种新的作用机制,在SCD中未被描述。

镰状细胞病(SCD)的线粒体功能发生改变,部分原因是一氧化氮(NO)的生物利用度低。精氨酸是产生NO的底物,在伴随血管闭塞疼痛事件(VOE)的SCD患者中变得严重缺乏。

为确定精氨酸是否能改善线粒体功能,Morris等人将12例因VOE住院的SCD患儿(年龄13.6±3岁,男性67%,HbSS 75%)随机分成3组(每组4人),接受不同剂量的精氨酸治疗:

    1)100 mg/kg静脉滴注,3次/天;

    2)负荷剂量200 mg/kg,然后100 mg/kg,3次/天;

    3)负荷剂量200 mg/kg,然后持续输注 300 mg/kg/天,直到出院。

分别检测患儿入组和出院时的富含血小板的血浆线粒体活性、蛋白表达和蛋白羰基。

结果显示,与稳定队列相比,所有VOE患儿在入组时的复合物V的活性显著降低。此外,3种精氨酸剂量治疗的受试者在出院时的复合体V的活性明显升高;当使用负荷剂量时,增加幅度最大。虽然VOE患儿的血小板复合体IV和柠檬酸合成酶的活性与稳定状态的相似,但VOE患儿在精氨酸负荷剂量治疗后酶活性显著增加。除此之外,精氨酸治疗还降低了所有患儿的蛋白质羰基水平,表明氧化应激有所减轻。

综上所述,本研究表明精氨酸治疗可增强SCD/VOE儿童的线粒体活性,减少氧化应激。
