前列腺癌(PCa)是全世界男性中最常见的恶性肿瘤之一。尽管局部PCa患者的5年生存率接近100%,但转移性前列腺癌仍然无法治愈。因此,迫切需要相关的标记物来检测监测肿瘤转移的初始阶段、肿瘤复发的可能性并预测转移的首选部位,以便对患者进行个性化治疗。

既往研究显示,上皮间质转化(EMT)和可塑性与PCa的转移相关。此外,EGFR(表皮生长因子受体)也被证实在前列腺癌的发生发展过程中起着至关重要的作用。目前已有证据显示,EGFR的表达水平与前列腺特异性抗原(PSA)重现、骨转移的高风险以及肿瘤的恶性程度相关。然而,EGFR的表达与PCa的特征和PCa转移的复杂关系还有待进一步的研究。

EGFR在原发肿瘤中过表达

该研究在1039例原发肿瘤,39例达米科高危患者的循环肿瘤细胞和21例去势抵抗性PCa病例的转移样本中追踪EGFR的过表达状态(EGFRover)。研究人员检测到14%的原发肿瘤样本中EGF处于过表达状态,对应着较短的无转移生存期,且这也是整体生存期较差的一个独立指标。

EGFR在癌转移中过表达

进一步研究发现,EGFR过表达状态与肿瘤细胞迁移前和转移前表型以及丰富的胶原纤维含量相关。所有循环肿瘤细胞(13%的病例中检测到的)均检测到EGFR阳性,该结果独立于它们的EMT表型。EGFRover在去势抵抗性骨转移瘤中更为普遍(占患者的29%)。


综上研究显示,EGFR的过表达状态是前列腺癌向骨骼转移的一种稳定的、独立于EMT作用的标记。
