Circulation:房颤基因风险预测研究

结果:1032(5.5%)例患者发生房颤。校正临床危险因素后,房颤的遗传风险评分与新发房颤相关。房颤事件发生的基因危险分数的最高四分位数VS最低四分位数的HR值, 1.28(719个变量;95% CI,1.13-1.46;P = 1.5×10 -4)到1.67(25个变量;95% CI,1.47-1.90;P = 9.3x10-15)。不同的研究和分数,临床和遗传风险评分也不相同(最大C统计,0.629-0.811;临床评分的最大ΔC统计,0.009-0.017)。在年龄和性别校正模型中,房颤的遗传风险与卒中相关。例如,在127个变量的最高VS最低四分位数,心源性脑卒中发生的几率增加2.49倍(95% CI,1.39-4.58;P = 2.7x10-3)。排除已知的AF患者(n = 70),有相同的结果(OR,2.25;95% CI,1.20-4.40;P = 0.01)。

结论:临床房颤危险因素除外,综合房颤遗传风险评分与房颤的发生相关,虽然区分度仅有小小的改善。在年龄和性别校正分析中,AF遗传风险还与心源性卒中相关。需要进一步确定AF遗传风险是否可以改善识别亚临床AF,或者可以帮助区分卒中发生的机制。
