PB:槐杞黄颗粒减轻环磷酰胺诱导肾毒性的机制研究

环磷酰胺(CYP)是一种烷基化抗癌药物,也会导致严重的肾毒性,槐杞黄颗粒可有效改善这种肾毒性,可考虑临床应用。

环磷酰胺(CYP)是一种烷基化抗癌药物,也是一种有效的免疫抑制剂,自20世纪50年代以来广泛用于治疗恶性肿瘤,如实体肿瘤和淋巴瘤,以及非肿瘤性疾病如类风湿关节炎和系统性红斑狼疮。然而CYP的应用也会导致严重的肾毒性,限制了它的临床应用。

既往研究发现CYP诱导的肾损伤与氧化应激、凋亡和炎症有关。此外,核苷结合域(NOD)样受体蛋白3 (NLRP3)炎性小体在包括肾损伤在内的各种炎症疾病的发展中发挥着重要作用,是一个潜在的治疗靶点。因此,调控这些病理生理过程的方法可能具有降低CYP肾毒性的潜力。

槐杞黄颗粒(HQH)是一种典型的中草药复方,主要由槐耳菌质、枸杞子、黄精组成。有临床试验报道,HQH能有效降低轻度IgA肾病患者的蛋白尿和血尿;体外研究表明,HQH可以通过减缓上皮-间质转化来保护肾小管上皮细胞等。值得注意的是,HQH对正常的肾脏细胞几乎没有毒性,可以长期使用。因此,HQH很有可能成为临床上治疗肾脏疾病的辅助药物。但是HQH在CYP诱导的肾毒性方面的作用仍不清楚。

近期在PHARMACEUTICAL BIOLOGY发表的一项研究,腹腔注射CYP诱导大鼠肾损伤模型,通过检测肾功能指标、形态学改变、氧化应激、细胞凋亡和炎症介质,分析NF-kB/MAPK通路的磷酸化和NLRP3炎症小体的激活,探讨HQH对CYP所致肾损伤的保护作用及其可能的分子机制。

结果显示:

  • HQH能降低CYP大鼠血清肌酐、尿素氮和尿蛋白水平,改善组织病理学异常。
  • HQH可降低CYP处理后MDA的生成,提高抗氧化酶CAT和SOD的活性
  • HQH通过调节凋亡相关蛋白(caspase-3、caspase-9、Bax、Bcl-2)和抑制炎症反应(IL-1b、IL-6和TNF-α)来改善CYP诱导的肾毒性。
  • HQH可以减弱经过CYP处理的大鼠的NF-kB/MAPK通路的磷酸化和NLRP3炎症小体的激活。

综上,HQH能有效改善CYP诱导的肾毒性,这种毒性与调节氧化应激、细胞凋亡和炎症有关,因此HQH可能是治疗CYP所致肾毒性的一种有效药物,可考虑临床应用。但该研究尚存不足,只关注了HQH对CYP所致肾毒性的保护作用,但其作用成分尚不明确;此外尚不清楚上游调控机制,有待进一步研究
