JAMA:PHQ-2单独或联合PHQ-9用于抑郁症诊断的准确性

相比于单独以PHQ-9得分大于10,以PHQ-2得分大于2 且PHQ-9得分大于10作为抑郁症的诊断标准可保证诊断敏感性的同时提供特异性。

PHQ-9是评估抑郁和抑郁严重程度的重要工具,而PHQ-2是PHQ-9的重要组成,评估了疑似患者兴趣及快感下降,情绪低落及无望水平,近日研究人员考察了单独使用PHQ-2或联合使用对PHQ-9抑郁评估的准确性。
 
本次研究为系统综述及荟萃分析,收集2000-18年间相关资料,将PHQ-2评分与经过验证的诊断性访谈得出的抑郁症诊断结果进行了比较,其中PHQ-2评分范围为0-6,PHQ-9评分范围为0-27。
 
100项研究,包含44318名参与者,其中4572名抑郁症患者(10%),患者平均年龄49]岁,59%女性。在使用半结构式访谈的研究中,PHQ-2得分2或更高用于抑郁症诊断的敏感性和特异性分别为0.91和0.67,截止分数为3或更高的为0.72和0.85。在不同类型的访谈中,特异性没有显著差异。半结构化访谈诊断的工作特性曲线下面积为0.88,完全结构化访谈为0.82。半结构化访谈中,PHQ-2评分大于等于2分,PHQ-9评分大于等于10分(0.82)的诊断敏感性与单独的PHQ-9评分大于等于10分(0.86)相比无显著性差异,但特异性增加但不显著(0.87 vs 0.85)。
 
研究认为,相比于单独以PHQ-9得分大于10,以PHQ-2得分大于2 且PHQ-9得分大于10作为抑郁症的诊断标准可保证诊断敏感性的同时提供特异性。
