NEJM:镰状细胞贫血羟基脲治疗剂量差异研究

对于撒哈拉以南非洲镰状细胞贫血儿童,升高剂量羟基脲治疗方案的临床效果优于固定剂量方案

已证明,羟基脲在撒哈拉以南非洲镰状细胞贫血儿童中具有较高的安全性、可行性和有效性,可降低血管闭塞发生率和死亡率。 然而,羟基脲疗法的剂量标准仍未确定,提高剂量对临床的影响尚不清楚。
 
近日研究人员比较了20mg/kg体重每日(固定剂量组)或最高30mg/kg体重每日(升高剂量组),这2个剂量的临床差异。研究的主要终点为24个月后血红蛋白水平为每升9.0g或以上或胎儿血红蛋白水平为20%或以上。次要终点为疟疾、血管闭塞性事件和严重不良事件。
 
94名儿童接受固定剂量(平均年龄4.6岁),93名接受高剂量(平均年龄4.8岁),羟基脲的平均剂量为19.2mg/kg和29.5mg/kg。试验结束时,升高剂量组86%的患儿达到了剂量阈值,而固定剂量组仅37%的患儿达到了剂量阈值。升高剂量组患儿的镰状细胞相关不良事件(0.43)、血管闭塞性疼痛(0.43)、急性胸部综合征或肺炎病例(0.27)、输液(0.30)以及住院(0.21)事件的发生率均较低。组间实验室认定的剂量相关毒性事件发生率相近,未发生严重中性粒细胞减少或血小板减少事件。
 
研究认为,对于撒哈拉以南非洲镰状细胞贫血儿童,升高剂量羟基脲治疗方案的临床效果优于固定剂量方案。
