JCEM:血浆甘油三酯和白细胞介素6水平与体重反弹和脂肪量增加有关

减肥成功后的体重反弹预防是超重或肥胖人群面临的最大挑战之一,因为减肥后的体重反弹几乎不可避免。研究表明,减肥维持可改善胰岛素抵抗、血脂异常、高血压、睡眠呼吸暂停等合并症,而体重反弹可轻易抑制这些益处,甚至再次诱发不良疾病。

近日,内分泌和代谢性疾病领域权威杂志Journal of Clinical Endocrinology & Metabolism上发表了一篇研究文章,在这项研究中,研究人员旨在调查在Yoyo饮食干预的随访期间,脂肪细胞大小的变化和其他危险生物标志物与体重反弹之间的关系。

在这项随机对照研究中,48名超重/肥胖参与者接受了(非常)低热量饮食来减肥,随后是4周的体重稳定期和9个月的随访期。研究人员分析了随访开始和结束时的人体测量指标、腹部皮下脂肪组织脂肪细胞体积和血浆代谢参数[游离脂肪酸(FFA)、甘油三酯(TG)、总胆固醇、葡萄糖、胰岛素、HOMA-IR、IL6、ACE活性、视黄醇结合蛋白4(RBP4)]。

研究人员分析发现TG、IL6、HOMA-IR、ACE的变化与体重反弹显著正相关。多元线性回归分析显示,只有TG和IL6的变化与体重反弹和体脂量增加保持显著相关。此外,HOMA-IR的变化与TG水平的变化密切相关。令人惊讶的是,随访期间脂肪细胞体积的变化与体重反弹及其他因素均无相关性,但在随访开始和结束时,研究人员发现脂肪细胞体积与HOMA-IR之间呈正相关。

由此可见,这些结果提示TG和IL6通过不同的机制与体重反弹相关,HOMA-IR和脂肪细胞体积可能通过血浆TG水平的变化与体重反弹之间间接相关。
