近日,呼吸疾病国家重点实验室赖克方教授课题组邓政博士的研究论文“Interferon-γ Enhances the Cough Reflex Sensitivity via Calcium Influx in Vagal Sensory Neurons”被“American Journal of Respiratory and Critical Care Medicine”正式在接收并在线发表。

近日,呼吸疾病国家重点实验室赖克方教授课题组邓政博士的研究论文“Interferon-γ Enhances the Cough Reflex Sensitivity via Calcium Influx in Vagal Sensory Neurons”被“American Journal of Respiratory and Critical Care Medicine”正式在接收并在线发表。

通过整体豚鼠咳嗽信号分析技术、原代培养大鼠迷走神经元、细胞免疫荧光成像、钙成像技术与膜片钳技术等方法,该研究首次探讨了IFN-γ对咳嗽敏感性的影响及其机制。研究发现IFN-γ吸入可以增加豚鼠咳嗽敏感性,通过JAK/STAT-PKA-AMPA信号通路引起迷走神经元钙离子内流,使膜电位出现去极化改变,从而提高咳嗽敏感性;当膜电位去极化程度达到快钠通道的阈电位时,就会产生动作电位。该动作电位的形成可能与咳嗽反射存在直接联系。

咳嗽敏感性增高是慢性难治性咳嗽的重要病理生理与临床特征,又称咳嗽高敏综合征。目前机制未明,尚无有效的治疗药物。本项研究首次证实IFN-γ在咳嗽敏感性中的作用,进一步阐明了咳嗽高敏感性的分子机制,并有望为慢性难治性咳嗽或咳嗽高敏综合征的治疗提供新的靶点。

2017年1月初着凉后出现咳嗽伴少量白粘痰,无发热、气促等不适。因原有哮喘史,故予舒利迭、普米克都保治疗,但无缓解,遂于1月中旬至当地医院予左氧氟沙星0.5g qd+甲强龙 40mg qd静滴,4天后症状好转。停药3天后再次出现咳嗽咳痰,予莫西沙星0.4g qd+ 甲强龙 40mg qd静滴,5天后好转。

探案:持续发热咳嗽1月余,突发呕吐引出罕见病!

2017年12月初开始发热,Tmax 40℃,伴咳嗽咳痰,黄脓痰,不易咳出,夜间盗汗明显。12-7至当地县医院,查血结果未见,拍胸部CT:右肺中叶大片炎症,左肺下叶基底段团片影,似见空洞,予以头孢哌酮/舒巴坦2g q12h+左氧氟沙星0.4g qd抗感染治疗共14天。期间12-14随访胸部CT :右肺中叶炎症范围较前缩小。治疗后患者体温降至正常,予以出院。
